IAOM 2021 Session 07 | Collaboration Care for Complex Cases
This presentation will discuss various professional roles in establishing collaboration care for complex cases. Information will include report evaluations, treatment plans, and ongoing consideration as progression occurs.
1. Explain the benefit of a multi-discipline team for patient management.
2. Review and identify patients who may benefit from a multi-disciplinary approach.
3. Apply referral criteria to identify patients who may require additional support to further access specialists to improve therapeutic outcomes.

45 minutes Case study assessment, documentation, maintenance, overall goals
